FAQ's

How do I know if my referral has been accepted into the PCPP?

All referrals received by the PCPP will be reviewed and triaged. Once the referral has been accepted by the team, you will receive a phone call with an appointment date. A package with intake forms and questionnaires will also be sent to you. Your referring physician/nurse practitioner will also be notified that an appointment has been given. If our program is not an ideal fit for your child’s condition we will notify the referring provider.

What will happen at the first appointment?

The first appointment involves meeting with the interdisciplinary team. Families often tell us it is easier to explain their story once so we believe that it is important that we meet together to listen to the concerns that you have in respect to past and present experiences in managing chronic pain. The assessment will include meeting with a physician or nurse practitioner (including a physical exam), a psychologist, a rehabilitation clinician and other members of the team.

Please bring:

  • Ontario Health Insurance Card (OHIP)
  • Any important health information such as: test results, CT scans, X rays, psychological assessment reports or other information that you would like to share with us
  • List of current medication and medications previously used (including dose/frequency)  (Some families find it helpful to ask for a medication list “print out” from their pharmacist)
  • Completed questionnaires (youth and family)
  • Appropriate clothing (tank tops/shorts) to make the physical assessment easier

This appointment will be up to 3 hours in length.  We would encourage you not to bring other children/siblings to this appointment.

How early should I come for my appointment?

Please come a half-hour early for your appointment. This will allow for sufficient time to park and for some additional paperwork needed upon your arrival.

What if I cancel this appointment?

If you cannot attend your scheduled appointment time, please contact the clinic as soon as possible to reschedule.  We will do our best to reschedule your appointment but it will be based on what appointments are available at the time. Three consecutively cancelled appointments will require reassessment of your need for services.
