What services do we provide:
Surgical and occupational therapy consultation and follow-up for infants and children with obstetrical brachial plexus injuries.
Urgent referrals - contact your family doctor or pediatrician or go to your nearest Emergency Department.
Do not fax urgent referrals
Referrals should be sent preferably within 2 weeks of birth, regardless of apparent severity, or as soon as possible therafter.
We will notify the doctor once the appointment has been scheduled.
Patients are seen by the surgeon on the 2nd and 4th Friday mornings of the month. An occupational Therapist will be in touch with the family prior to begin therapy, as required.
The family will be notified if an earlier appointment is required or becomes available.
Changing your appointment:
Please contact the clinic as soon as possible, if you need to change or cancel your child's appointment.
If your child has a communicable disease, such as Chicken Pox, or has been in contact with someone with a communicable disease, please call the clinic before you come to the hospital.
What you should bring to the clinic:
Please bring your child's curent Ontario Health Insurance Card. The Ministry of Health requires us to validate your Health card upon arrival.
All birth records and any test results or assessment reports available from other institutions.
Any notes or questions that you may have.

The Family Resource Library is located in the 3F clinic (red section, 3rd floor). Volunteer staff are available to help families find information about their child's health or illness. Pamphlets, books, and other resources on the internet are available.
Every six months we survey families to ask your perceptions of care. If you have any concerns, please contact Paul Agar, Clinical Manager.
